This position is a contracted 1099 position, working with individuals with disabilities to achieve their employment goals through vocational rehabilitation.

$500 sign-on bonus for full-time and $250 sign-on bonus for part-time after 90 days of employment __________ Position Objective Provides job placement assistance to program participants with barriers to employment, and to provide and maintain a positive image in the community by creating or participating in community events Essential Duties and Responsibilities Provides services leading to the improved productivity skills of participants enrolled in the Community Employment Program through intervention with the participant and coordination with the participant's supervisor.
Conducts intake interviews and provides vocational counseling to persons served in the Community Employment Program, focusing on work habits, interpersonal skills, and other behaviors and issues impacting work performance.
Provides intervention as needed in the areas of the participant's life with a main goal of enhancing vocational and career success and a secondary goal of improving the participant's quality of life in other areas.
Assists each participant in gaining Community Employment.
Implements and monitors progress in achieving employment goals.
Assists in securing other placement sites, when appropriate, and assists with transition to alternative employment.
Follows the Certified Rehabilitation Counselor Code of Ethics.
Participates as part of an interdisciplinary team in the development of each client's Individual Support Plan (ISP) and Person Centered Plan (PCP).
Complete daily, weekly, monthly, and yearly documentation as required by Sky Point and state and federal regulations.
Audit files to make sure they meet standards according to standards and protocol established.
Communicate professionally with families, Case Manager, Behavioral Consultant, health care personnel, schools, landlords, employers, etc Participate in Sky Point's Managers meetings, team meetings and other departmental meetings, ISP, Quarterlies, IEP meetings, and other meetings as assigned.
Implement required training protocol for new and existing staff.
Respond appropriately and promptly to on-call situations.
Report any suspected abuse, neglect, or exploitation to the supervisor or department head.
Maintain employment-required documentation (current auto insurance, driver's license), certification (i.
e.
, First Aid, CPR, Medication Administration, Mandt, etc.
), and training (OSHA, Abuse & Neglect, etc.
) as required to comply with federal, state and program requirements and standards.
Must maintain good attendance throughout employment.
Comply with all standards to assure the health and safety of all individuals.
Report to work as scheduled.
Performs other duties as assigned.
Qualifications and Education:
High School Diploma or GED required; Associates Degree or higher preferred with 2 years comparable work experience.

Essential Job Requirements/Functions:
Employee must be at least (18) years of age.
Employee must be able to read and write proficiently.
Employee must have and maintain an Indiana driver's license, and proof of a good driving record.
Employee must be able to successfully complete all required training in the required time allotted.
Physical Demands:
Ability to travel by automobile to various worksites for conducting business is necessary.
Must be able to communicate well orally and in writing with board of directors, top management, legal staff, and coworkers and contractors individually and in groups.
Regular use of a telephone, email, or other medium to communicate is essential.
Sitting for extended periods is common.
Hearing and vision within normal ranges is essential for normal conversations, to receive ordinary information, and to prepare or inspect documents.
No heavy lifting is expected.
Exertion of up to 10 lbs.

Good manual dexterity required for the use of common office equipment such as computer terminals, calculators, copiers, and FAX machines.
Good reasoning ability and an eye for detail are required to solve a wide range of business problems.
